Pre-adoption survey

  • The pre adoption survey is a guide to help us determine if an animal will be the right fit for you and your family. Based on the answers to the questions below, we reserve the right to deny any adoption. Our goal is to make the right fit for your family and the pet.

    ► All adopters must be at least 18 years of age.

    ► We cannot hold animals.

    ► Adoptions are not finalized until the adopter comes to the shelter to meet the animal and pays any fees involved with the adoption.

    ► Completing this form is not an adoption contract and does not obligate you to adopt.
