  • Congratulations on the adoption of your new pet!

We will contact you by phone or email to schedule the date and time that you can come to the shelter to pick up your new pet.

Before your scheduled pick up date, you need to complete the short form below so that we can document that you have viewed the pet post-surgery discharge and pet safety information. This video must be viewed before you pick up your animal.

We also have a helpful list of supplies and information for you to help get started with your new friend!

  • Please view the discharge slide show that follows. It contains important information on caring for your new pet.

  • I have reviewed the discharge information and understand what is required of me as a pet owner after an adoption.

Additionally, I understand that I should not bring more than one other person with me to pick up the pet, nor should I bring any owned pets.
